The ACCESS Model is a voluntary 10-year CMS Innovation Center model designed to expand access to technology-supported care for people with chronic conditions in Original Medicare. It tests outcome-aligned payments to support more integrated, patient-centered care while improving health outcomes, patient choice, and overall Medicare value.

MAHA ELEVATE is a CMS Innovation Center initiative focused on evaluating evidence-based functional and lifestyle medicine interventions for Original Medicare beneficiaries. The model will fund programs that collect quality and cost data on whole-person approaches intended to support chronic disease prevention, health promotion, and healthier aging.

Tech-Forward PopHealth

Tech-Forward Population Health is centered on the use of digital health tools and interoperable data infrastructure to improve outcomes, expand access, and support more coordinated care. It aligns with CMS’s broader focus on telehealth, remote monitoring, and FHIR-based information exchange to advance more connected, patient-centered, and data-driven care.
